Our Locations

For the SLO office, the  parking lot for 5 Star Medical Inc. is accessed only via El Tigre Ct.

At the intersection of Los Osos Valley Road and Madonna Road, drive on Madonna Road and turn right on El Tigre.

El Tigre dead ends into a cul de sac and the last building on the right is 11545 Los Osos Valley Road. 5 Star Medical Inc. It is on the first floor. Ryan Ross DDS is also in the building. If you get lost, please call the number below.

We are next to the old Sesloc Credit Union Building, but a fence divides us.

For the Santa Maria office, we are in the InSparations building located at 130 E. Clark Ave, Old Orcutt, Ca.

It’s a gray building resembling a Salon with the house number on the right hand side.

From SLO, take the 101 south turn right on Clark Ave, go approximately 2.5 miles and the building will be on the left hand side.

For the Atascadero office, we are the building in between McDonald’s and Brew it Coffee. We are located at 6500 Morro Rd, Suite A
